In order to best serve you, please complete the appropriate questionnaire. The completed questionnaire must be returned to our office, along with supporting documentation, at least one week prior to your scheduled appointment. If your completed questionnaire and supporting documents are not received one week before your appointment, it is our office policy to coordinate rescheduling your appointment to another mutually convenient time.
Please know that completing and forwarding a questionnaire does not in and of itself establish an attorney-client relationship. An attorney-client relationship is only established once an engagement agreement with our firm is signed.
